LPN Referral Management PRN VARIED Shifts

Responsibilities

LPN Referral Management PRN will assess and coordinate care of patients referred for Mental Health Services, reviewing referral information, determining appropriateness of services, and facilitating connection between patients and appropriate facilities or treatment programs. This role ensures patients receive timely and effective care tailored to their specific behavioral health needs.

• Review assessment referrals received and collaborate with RN to assess patient needs.
• Assist with coordination of patient care.
• Maintain patient records, update databases, and ensure accurate documentation of patient information.

EDUCATION
:
Nursing Degree (preferred Bachelor’s or Master’s).

LICENSURE/CERTIFICATION
:
Licensed LPN in current state or compact license. Current CPR if required by a customer hospital.

EXPERIENCE
:
Preferred 1-3 years related work experience in behavioral health.
