Terros Health is a health care company focused on the whole person, providing primary care and specializing in mental health and substance use treatment. The Family Nurse Practitioner will work at the Priest Drive Health Center in Tempe, AZ.
Responsibilities- Performs independently or as part of a treatment team.
- Conducts physicals/check‑ups, evaluations, diagnostics, treats illnesses, orders lab tests, provides general and preventative care, prescribes and monitors medications, and conducts individual treatment and case consultation.
- Educates patients about preventative care, wellness activities, and prescribed treatments.
- Provides leadership and cohesion within the clinic environment and may provide direct supervision to other professional and/or non‑professional medical staff and students.
- Participates in program evaluation procedures and professional record‑keeping, and makes referrals appropriately for specialty and behavioral health care.
- Provides direct patient services for at least 85% of the work time.
- Employs therapeutic interventions appropriate for identified treatment issues.
- Seeks appropriate clinical supervision/support when necessary to ensure effective treatment.
- Establishes appropriate patient files that conform to agency guidelines, best professional practice, and state licensing regulations.
- Active Nurse Practitioner license in Arizona.
- Master’s degree or Doctorate in nursing (Family Nurse Practitioner).
- Valid BLS certification.
- Valid DEA license and Board Certification (ANCC).
- Valid Arizona driver’s license, minimum 21 years of age, and at least 4 years driving experience; must meet Terros Health’s driving policy.
- Valid Arizona Fingerprint Clearance card or must obtain one within 7 working days of assuming the role.
- Pass a drug screen, TB test, and background check.
- Proficient using computer‑based systems including electronic health records (Next Gen preferred), Microsoft Outlook, and other Microsoft Office products.
