LPN Referral Management PRN – 12 Hours Varied Shifts

Behavioral Health Connections, Inc. (BHC) is a provider of patient‑focused, comprehensive mental health services. We offer mental health evaluations to children, adolescents, adults and older adults in crisis 24 hours a day, 7 days a week, including all holidays. Our service model focuses on the assessment, diagnosis, treatment and disposition of mental health patients. Based on the results of an assessment, we determine the most appropriate level of care and recommend treatment facilities.

Our tele‑psychiatric assessment and consultation services connect patients with the best equipped providers to address their mental health issues. Rapid response ensures patients receive timely and effective care and expedite their journey to wellness.

LPN serves to assess and coordinate the care of patients referred for mental health services. The role reviews referral information, determines the appropriateness of services, and facilitates the connection between patients and the most appropriate facilities or treatment programs. This is crucial in ensuring patients receive timely, tailored behavioral health care.

• Review assessment referrals received and collaborate with RN to assess patient needs.

Education: Nursing Degree; preferred Bachelor’s or Master’s.

Licensure / Certification: Licensed LPN in the current state or compact license; current CPR if required by a customer hospital.

Experience: Preferred 1‑3 years related work experience in behavioral health.
