Relive Health Tempe is a growing health and wellness practice focused on helping clients feel, perform, and live better through proactive, personalized medicine.

Our services include hormone optimization, medical weight loss, peptide therapy, IV therapy, regenerative medicine, and advanced wellness diagnostics. We take a relationship-based approach to care, giving our providers the opportunity to understand each client’s goals, evaluate their health comprehensively, and develop personalized treatment plans.

We are looking for an experienced and highly motivated Nurse Practitioner who is passionate about hormone optimization, functional medicine, medical weight loss, and preventative health.

This is an ideal opportunity for an NP who enjoys spending time with clients, interpreting comprehensive lab work, solving complex health concerns, and helping people achieve measurable improvements in how they feel and function.

We are particularly interested in candidates with experience in TRT/HRT, GLP-1 medical weight loss, peptide therapy, and functional or integrative medicine.

• Conduct comprehensive client consultations and health assessments
• Review and interpret detailed laboratory results
• Develop and manage individualized hormone optimization programs, including TRT and HRT
• Manage medical weight-loss patients, including GLP-1 therapies
• Recommend appropriate peptide, supplement, IV, and lifestyle protocols
• Monitor client progress and adjust treatment plans based on symptoms, labs, and outcomes
• Educate clients so they understand both their health concerns and treatment options
• Build trusted, long-term relationships with clients
• Collaborate closely with our medical team, nurses, and client advocates
• Maintain accurate EHR documentation and follow all applicable clinical and regulatory standards

• Active and unrestricted Arizona Nurse Practitioner license
• Ideally 1-3 years of clinical experience
• Experience with hormone replacement/optimization, TRT, medical weight loss, or functional/integrative medicine
• Strong ability to interpret laboratory results and translate findings into individualized treatment plans
• Familiarity with peptide therapy and nutritional supplementation is highly desirable
• Excellent communication and patient-education skills
• Comfortable developing long-term relationships with clients rather than simply treating individual symptoms
• Collaborative, personable, and committed to delivering an exceptional client experience
