Spanish PreferredGENERAL DESCRIPTION Performs advanced and complex casework in working with individuals participating with the Youth Empowerment Services Team. Work involves serving as a liaison between clients and their families in obtaining assistance, accessing community support resources, and providing wraparound intensive case management reduce out of home placements. Work under general supervision with limited latitude for the use of initiative ad independent judgement.
ESSENTIAL DUTIES AND RESPONSIBILITIES Interviews individuals, their families, other agency members and other Interdisciplinary Treatment Team members to obtain client background information and to identify the psychosocial, economic, and physical needs of the individuals.
Assesses individuals’ current and ongoing needs with regards to support systems and available community resources, schools, and facilities, etc., as needed and requested by the individual.
Identifies problem areas, service gaps, and areas that are barriers to services for clients, providing wraparound intensive case management and referral to YES Waiver contract services.
Serves as a liaison between clients and their families, state and judicial systems, and facility or treatment providers.
May participate as an active member of an Interdisciplinary Treatment Team.
Participate in Monthly Family Team Meeting May make recommendation on YES policy and procedure changes.
Provides Crisis Intervention Services as needed.
Assists with developing individual Wraparound recovery plans, and in monitoring the observance of clients’ rights and service provision as identified in the recovery plans.
Meets all clinical needs and minimum service requirements according to Center staff training requirements, Resilience & Disease Management Design and Texas Administrative Codes.
Participates in quarterly training and time studies for Random Moment Time Study/Medicaid Administrative Claiming (MAC).Document case records, assessments, Wraparound recovery plans and progress notes according to Center requirements and Texas Administrative Codes. Employees will maintain complete and accurate consumer medical records and keep all such information confidential within two business days.
Documentation – This function requires that the employee clearly and legibly document services provided to include ongoing progress notes, consultation notes, correspondence. All chart documentation will be turned in by the established deadline 100% of the time. In addition, employee time sheets, travel logs, and other employee and CMH child documentation will be turned as required.
Work behavior must be compatible with Center value statement and Policies and Procedures Manual.
Performs related work as assigned by supervisor.
Demonstrate high ethical standards, trustworthiness, and personal integrity. Regular attendance, dependability, and promptness are required for the 8-5 work day or agreed upon hours 100% of the time, to ensure consistency and completeness of program’s processes.
Professional Development
- Successfully complete training required to maintain skill competency and cross training to ensure consistency across program procedures. Complete continuing education as required by professional standards and the Center.
Must stay compliant with all required training.
GENERAL QUALIFICATIONS Experience/Education/Licensing/Certification Experience in casework.
Graduation from an accredited four-year college or university with a major course work in a field relevant to the assignment is required.
Experience and education may not be substituted for one another.
