Enhabit Home Health & Hospice in Temple, TX, is seeking a coordinator to schedule patients to branch field clinicians and ensure seamless care coordination. You will communicate with field staff, patients, physicians, referral sources, caregivers, and other providers to maintain continuity of care.
This role requires a high school diploma, and either current LPN/LVN licensure in Texas with clinical experience or at least one year in home health, hospice, or pediatrics.
