Role Overview

As a Home Health LPN/LVN, you provide direct patient care under the supervision of a Registered Nurse, helping to implement care plans and ensure patients receive safe, effective treatment in their homes.

You are a key part of the care team – delivering treatments, monitoring patient conditions, and reinforcing education to support successful outcomes.

What You’ll Do
  • Provide direct patient care in accordance with the state Nurse Practice Act.
  • Implement and support the plan of care developed by the RN.
  • Deliver skilled nursing services, including treatments, medication support, and wound care.
  • Monitor patient condition and report changes to the RN and care team.
  • Educate patients and caregivers on self‑care, treatments, and disease management.
  • Maintain infection control and proper use of equipment and supplies.
  • Document patient care accurately and timely within the EMR.
  • Communicate effectively with physicians, nurses, and interdisciplinary team members.
  • Participate in case conferences, team meetings, and ongoing training.
  • Support coordinated care by maintaining consistent communication across the care team.
What You Bring
  • Active LPN/LVN license in the state of practice.
  • Graduate of an accredited practical/vocational nursing program.
  • Current CPR certification.
  • Minimum one (1) year nursing experience preferred (Home Health, Med‑Surg, or Acute Care a plus).
  • Strong clinical and observational skills.
  • Clear communication and patient education abilities.
  • Ability to follow care plans while recognizing and reporting changes in condition.
  • Basic proficiency with EMR systems.
  • Valid driver’s license and reliable transportation.
