- Supportive clinical leadership and administrative team
- Conduct comprehensive nursing assessments and develop individualized plans of care
- Complete OASIS Start of Care, Resumption of Care, Recertification, and Discharge assessments
- Perform wound care and dressing changes
- Manage medication and educate patients
- Provide catheter, ostomy, and other skilled nursing care
- Teach disease management and chronic care education
- Coordinate care with physicians and interdisciplinary healthcare teams
- Monitor patient progress and identify changes in condition
- Order medical supplies as needed
- Complete accurate, timely electronic documentation

Requirements- Current Massachusetts Registered Nurse (RN) license
- Minimum of one (1) year of clinical nursing experience
- Afternoon availability
- Previous home care experience
- Monday–Friday availability
