Signature HealthCARE of Rockwood Rehab & Wellness Center is seeking an experienced MDS Nurse to lead resident assessments and coordinate the MDS process in a 157-bed facility. You will collect data from medical records, perform bedside assessments, and collaborate with an interdisciplinary team to ensure compliant assessments and care planning.
RN licensure, 3+ years of clinical experience (long-term care preferred), and 1+ year of MDS experience are required.
