Physical Therapist; PRN

Role

Summary:

The Physical Therapist provides age-appropriate hospice services, including assessments, direct treatment, development of home exercise programs, patient and family education, adaptive equipment evaluation, and thorough documentation in line with hospice standards.

• Conducts physical therapy evaluations based on physician orders and Plan of Care for symptom management and maintaining daily functional skills.
• Documents patient visits, reports reactions to treatment, and updates physicians and administrators through the Care Management process.
• Participates in the Interdisciplinary Group, offering input for the patient's Plan of Care.
• Instructs patients, families, and hospice team members on therapeutic activities and progress toward goals.
• Prepares discharge summaries when therapy services end, ensuring records are updated and shared appropriately.
• Provides guidance to patients and families on physical therapy techniques as needed.

• Current Physical Therapy licensure in the practicing state.
• Current CPR certification.
• Ability to work effectively in diverse home environments.
• Valid driver’s license, vehicle insurance, and access to transportation.
• Preferred:
  Strong multitasking, independence, communication, organizational skills.
