Member Duties
The Community Health Support Member (CHSM) will focus on working in the community to address problematic components of the Healthcare system. This Member will collaborate with medical providers, primary care teams, and other patient support agencies to best improve and cater to local community needs. This role will assist partners in providing health-related services, including facilitating information on local social services and medical facilities.
This CHSM will help patients to schedule appointments, utilize available resources, and assist in completing applications for relevant programs. This role will require the ability to plan, implement, and evaluate individual client care plans. The CHSM will document relevant activities, service plans, and medical results in an effective and confidential manner. In this role, the CHSM could be asked to develop community outreach programming to promote healthy habits, vaccination, or other similar healthy living initiatives.
