Licensed Practical Nurse; LPN Wellness Team

Presley Healthcare, LLC LPN Wellness Nurse (2 Pikeville Locations)

Presley Healthcare, LLC is looking for an experienced LPN to become our wellness nurse in our 2 Pikeville Locations. This position focuses on preventive measures and visits for Medicare patients along with patient outreach for assistance with medications and social determinants of health.

You will also be responsible for follow up of patients who have recently been hospitalized or in a long-term care facility, such as a nursing home.

• Follow up of patients recently hospitalized or in a long-term care facility.
• Preventive care visits for Medicare patients.
• Patient outreach for medication assistance and social determinants of health.

• Must be up to date on licensure and certifications including BLS.
• Experience in phlebotomy, IV therapy, and quality measures is a plus.
• Preferred applicants will have 2+ years experience in a clinical setting; recent graduates are welcome to apply.

Position is Monday - Friday day shift. NO ON-CALL. No weekends.

Job Type: Full-time

Pay: $18.00 - $20.00 per hour
