Location: Harriman
Community Eldercare Services, LLC is seeking a highly skilled MDS/Care Plan Coordinator in Harriman, TN. In this role, you will conduct nursing assessments, oversee documentation review, and ensure the MDS and care plans are accurately completed in accordance with established standards.
The ideal candidate has an Associate's or Bachelor's degree, valid LPN or RN License, and 2-4 years of relevant experience. Long-term care experience is preferred, and you will lead interdisciplinary meetings and assist with audits and compliance as necessary.
