- As a Home Health RN youll bring skilled compassionate care directly to patients by Performing patient assessments admissions and ongoing care planning
- Coordinating care with physicians and interdisciplinary teams
- Delivering therapeutic preventative and rehabilitative nursing care
- Monitoring patient progress and updating plans of care
- Supporting patients and families through all stages of care
- Maintaining accurate and timely documentation
- Active Registered Nurse RN license
- Graduate of an accredited nursing program
- Minimum 1 year of clinical experience ICU Med Surg or Rehab preferred
Home Health experience a plus - Current CPR certification
- Reliable transportation valid drivers license and auto insurance
