Optum is seeking a Home and Community Based Services Licensed Practical Nurse to deliver direct nursing care in patients’ homes under physician orders. You will record progress notes, coordinate services, and support high-quality patient outcomes within the assigned geographic area.
Qualified candidates hold LPN licensure, have a valid driver’s license, and demonstrate strong communication and organizational skills. CPR is preferred or can be completed within 90 days of hire.
