Registered Nurse (RN) – Home Health | Full-Time

Tennessee Quality Care is seeking a Full-Time Registered Nurse (RN) to join our Home Health team serving patients throughout Wayne, Lawrence, and Hardin Counties. If you're an experienced RN looking for more independence, flexibility, and one-on-one patient care, Home Health may be the change you're looking for. You'll have the opportunity to provide meaningful care in patients' homes while being supported by an experienced clinical team.

Home Health experience is preferred, but not required. RNs with clinical experience in Med-Surg, ICU, Rehab, Skilled Nursing, or other direct patient-care settings are encouraged to apply.

What You'll Do

  • As a Home Health RN, you'll provide skilled nursing care to patients in the comfort of their homes.

    Responsibilities include:
  • Completing comprehensive patient assessments
  • Performing admissions and ongoing nursing visits
  • Developing and updating individualized plans of care
  • Providing therapeutic, preventative, and rehabilitative nursing care
  • Monitoring patient conditions and communicating changes to physicians
  • Coordinating care with physicians, therapists, and other members of the interdisciplinary team
  • Educating patients and families regarding medications, diagnoses, treatments, and ongoing care
  • Completing accurate and timely clinical documentation
  • Helping patients safely recover and maintain independence at home

What We're Looking For

  • Active Registered Nurse (RN) license
  • Graduate of an accredited nursing program
  • Minimum of 1 year of clinical nursing experience
  • Med-Surg, ICU, Rehab, Skilled Nursing, or Home Health experience preferred
  • Current CPR certification
  • Valid driver's license, reliable transportation, and auto insurance
  • Strong communication, organization, and time-management skills
  • Ability to work independently while collaborating with the clinical team
