Registered Nurse - 7on​/7off - Hospice

Role: After hours Registered Nurse providing hospice care outside regular hours, including nights and weekends.

Responsibilities

• Triaging patient needs via phone until in-person visit can be performed
• Responding urgently to after hours crisis calls from patients and families
• Conducting initial nursing evaluations to determine hospice eligibility within 48 hours of referral
• Performing after hours visits such as admissions, death, recertification, and routine visits
• Assessing and reassessing patient and family physical, psychosocial, emotional, and environmental needs every 15 days
• Documenting issues, goals, interventions, and responses to care
• Providing care in diverse home environments, respecting all backgrounds

• Current unrestricted RN licensure in the practice state
• Valid driver’s license, vehicle insurance, dependable vehicle or access to transportation
• Preferred: CPR certification (or ability to obtain within 90 days) and 1+ years of clinical experience
