Location: Alpine

Signature HealthCARE is seeking a Licensed Practical Nurse to provide compassionate, resident-centered care in our long-term and post-acute communities. In this role, you will deliver hands-on nursing care, administer medications and treatments, monitor resident conditions, and document accurately in accordance with regulatory standards. You will collaborate with an interdisciplinary team, support CNAs, and educate residents and families. Responsibilities

  • Provide direct nursing care to residents, including medication administration, treatments, and monitoring vital signs
  • Assist with admission, assessment, care planning, and documentation in accordance with facility and regulatory standards
  • Collaborate with RNs, physicians, therapists, and CNAs to deliver coordinated, person-centered care
  • Educate residents and families on care plans, treatments, and health management
  • Respond to changes in resident condition, report concerns promptly, and assist in emergency interventions
  • Maintain accurate, timely clinical records and ensure compliance with policies and procedures
  • Support a resident-centered, faith-based culture of dignity, respect, and compassionate service
  • Supervise and support CNAs and other support staff to ensure quality and safety
  • Participate in quality improvement initiatives, training, and ongoing professional development
  • Promote infection control, safety, and regulatory compliance throughout the facility
