LPN/LVN Long Term Care/SNF
Active Tennessee or Compact LPN license required. Minimum 2 years of recent LPN/LTC experience required. Current BLS certification required. Previous LTC, SNF, or Skilled Nursing experience required. Strong medication administration, resident assessment, care planning, and clinical documentation skills. Experience caring for geriatric and long-term care residents preferred. Ability to recognize changes in resident condition and communicate effectively with the interdisciplinary team.
