Role Overview

The Home and Community Based Services Licensed Practical Nurse provides direct nursing care to acutely or chronically ill patients in their homes, under supervision.

• Deliver high-quality clinical services within scope of practice, adhering to state laws, care plans, and infection control protocols.
• Document patient visits thoroughly in electronic medical records and visit verification systems.
• Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members.
• Provide care in diverse home situations, respecting all clients regardless of background or condition.

• Current, unrestricted LPN licensure in the practice state.
• Valid driver’s license, vehicle insurance, or access to reliable transportation.
• Preferred: CPR certification, ability to work flexible hours, manage multiple tasks, work independently, and possess strong communication and organizational skills.
