Gentiva Hospice is seeking an After hours RN Care Manager to provide high-quality hospice care outside standard business hours, including nights and weekends from Friday 4pm to Monday 8am. You will assess needs, coordinate care, and maintain clinical oversight for assigned patients within the IDG framework.
You will lead visits, ensure documentation, and support regulatory compliance while educating families and collaborating with physicians.
