• Provide 1:1 care for clients following the client’s plan of care while maintaining dignity and independence.
• Administer medications, feedings, oxygen, and ostomy care as needed, based on the client’s plan of care and assessment results.
• Accurately document nursing actions and communicate with families, case managers, and physicians, documenting progress and outcomes for established goals.
• Coordinate or order supplies, medications, and feedings as needed to ensure adequate availability.
• Ensure all patient needs are met in accordance with physician instructions.

• LPN/LPT/LVN degree from an accredited nursing program with current unrestricted license in the state of practice and residence.
• Valid driver’s license, acceptable driving record, and proof of car insurance.
• CPR professional certification.
• Preferred: 1+ year nursing experience.
• Preferred:
  Home health care experience.
