Licensed Practical Nurse

Standing Stone Care and Rehab Center in Cookeville, TN is seeking a compassionate Licensed Practical Nurse to provide resident-centered care in our skilled nursing and rehab facility. The LPN will administer medications and treatments, monitor resident conditions, document accurately in medical records, and support RNs and physicians with care plans. Responsibilities include wound care, IV support per scope, coordination with therapy teams, and family communication.

Responsibilities

• Provide direct nursing care to residents in a skilled nursing and rehab setting
• Administer medications and treatments per physician orders and LPN scope
• Monitor, assess, and document resident conditions and vital signs
• Support RNs and physicians with care planning and clinical follow-up
• Perform wound care and basic IV support within scope and policy
• Ensure accurate, timely charting in electronic medical records
• Communicate with residents' families and interdisciplinary team members
• Follow infection control, safety, and regulatory standards
• Assist with admissions, discharges, and resident education
• Contribute to a compassionate, resident-centered, team-based culture
