Licensed Practical Nurse

Standing Stone Care and Rehab Center in Cookeville, TN is seeking a Licensed Practical Nurse to provide hands-on nursing care for short-term rehab and long-term care residents. The LPN will administer medications and treatments, monitor residents' conditions, document care, and support care plan development with the interdisciplinary team. This role includes collaborating with RNs, therapists, and CNAs to deliver resident-centered, compassionate care while promoting safety, dignity, and comfort.

The LPN will educate residents and families, respond to changes in condition, and contribute to quality improvement in a supportive, team-focused environment.

Responsibilities

• Provide direct nursing care to rehab and long-term care residents
• Administer medications and treatments per physician orders
• Monitor and document residents' conditions and vital signs
• Participate in developing and updating individualized care plans
• Collaborate with RNs, therapists, CNAs, and physicians
• Educate residents and families on care, medications, and procedures
• Respond to changes in resident condition and report promptly
• Maintain accurate, timely clinical documentation
• Promote resident safety, dignity, and comfort
• Support quality improvement and infection control initiatives
