Registered Nurse

Standing Stone Care and Rehab Center in Cookeville, TN is seeking a compassionate Registered Nurse to deliver high-quality clinical care to skilled nursing, rehab, and long-term care residents. The RN will assess residents, develop and update care plans, administer medications and treatments, monitor changes in condition, and coordinate with physicians, therapists, and families. The nurse will supervise CNAs, ensure accurate documentation, and uphold regulatory and infection-control standards.

Responsibilities

• Provide skilled nursing care to rehab and long-term care residents
• Assess residents and develop, implement, and update individualized care plans
• Administer medications, treatments, and therapies per provider orders
• Monitor, document, and report changes in resident condition
• Collaborate with physicians, therapists, and interdisciplinary team members
• Educate residents and families on care, medications, and health management
• Supervise and support CNAs and other care staff on assigned unit
• Maintain accurate clinical documentation and electronic health records
• Ensure compliance with regulatory, safety, and infection-control standards
• Participate in quality improvement and resident-centered care initiatives
