Position: LICENSED PRACTICAL NURSE
Location: Englewood

Sevita is seeking a Licensed Practical Nurse to deliver compassionate, person-centered care in our Healthcare & Medical Services team. In this part-time role, you'll provide direct nursing care, administer medications, monitor patient conditions, and collaborate with an interdisciplinary team to support individualized care plans. You'll educate individuals and families on health management while promoting dignity and independence. Responsibilities

  • Provide direct, person-centered nursing care to individuals in home and community-based settings within LPN scope of practice.
  • Administer prescribed medications and treatments safely and accurately, following physician orders and Sevita protocols.
  • Monitor, assess, and document vital signs, symptoms, and changes in condition, reporting concerns promptly to the RN or provider.
  • Assist with activities of daily living (ADLs) such as hygiene, grooming, dressing, and feeding as needed.
  • Perform basic wound care, catheter care, enteral feeding support, and other routine nursing procedures as appropriate.
  • Support individuals with mobility, transfers, positioning, and use of adaptive equipment to promote safety and independence.
  • Contribute to behavioral support and crisis prevention strategies in coordination with behavioral health staff.
  • Educate individuals and families on health conditions, medications, safety, and self-care techniques.
  • Maintain accurate, timely documentation in electronic health records, incident reports, and care notes in compliance with regulations.
  • Collaborate with RNs, physicians, therapists, case managers, and direct support staff to implement and update person-centered care plans.
  • Follow infection control, safety, and emergency procedures in all settings.
  • Travel between homes or community locations as scheduled, maintaining punctuality and reliability.
  • Participate in ongoing training, in-services, and professional development offered by Sevita.
