Registered Nurse

Spring City Care and Rehab Center seeks a compassionate Registered Nurse to provide high-quality care to short-term rehab, long-term care, and memory care residents in our skilled nursing facility. The RN will perform assessments, administer medications and treatments, oversee wound and IV care, and coordinate individualized care plans in collaboration with an interdisciplinary team. Responsibilities include supervising CNAs, accurate EHR documentation, and promoting resident dignity and safety.

Responsibilities

• Deliver holistic nursing care to short-term rehab, long-term care, and memory care residents
• Complete assessments, develop and update individualized care plans
• Administer medications and treatments safely and accurately
• Monitor, document, and report changes in resident condition using EHR systems
• Collaborate with therapy, medical providers, and support staff on care coordination
• Educate residents and families on care plans and health conditions
• Support a resident-first, dignified, and respectful environment
• Supervise CNAs and other support staff to ensure quality care
• Adhere to infection control, safety, and regulatory standards
• Participate in quality improvement and interdisciplinary team meetings
