Gentiva Hospice in Tennessee is hiring a Registered Nurse Case Manager to lead in-home hospice and palliative care. This role covers patient and family support, assessment, and care coordination within a multidisciplinary team.
The RN will conduct initial and ongoing assessments, document care plans, and educate caregivers. Work schedule 12pm-8pm, Monday-Friday with limited after-hours on-call rotation, allowing mornings free.
