Night Shift Licensed Practical Nurse

Signature HealthCARE seeks a Night Shift Licensed Practical Nurse to provide resident-centered nursing care in our long-term care and post-acute communities. The LPN will deliver compassionate bedside care, administer medications and treatments, monitor resident status, and accurately document in EMR systems. Working within an interdisciplinary, faith-based team, you'll support admissions, discharges, and care planning while mentoring CNAs and promoting dignity, safety, and comfort.

Responsibilities

• Provide resident-centered night shift nursing care in long-term and post-acute settings
• Administer medications and treatments per provider orders and facility policies
• Monitor, document, and report changes in resident condition using EMR systems
• Support admission, discharge, and care plan processes with the interdisciplinary team
• Perform wound care, basic IV therapy, and other skilled nursing tasks as permitted by license
• Collaborate with CNAs and other staff to ensure safe, dignified care
• Educate residents and families on care plans, treatments, and health management
• Uphold infection control, safety, and regulatory standards during all shifts
• Participate in shift handoffs, audits, and quality improvement initiatives
