Position: LICENSED PRACTICAL NURSE
Location: La Follette

Cumberland Village Center, part of Genesis Healthcare, is seeking a Licensed Practical Nurse to join our Healthcare & Medical Services team. In this role, you will provide direct nursing care to elderly patients in a skilled nursing setting, supporting holistic wellness, rehabilitation, and safety. Collaborate with an interdisciplinary team, administer medications and treatments, monitor patient conditions, and document care accurately. Responsibilities

  • Provide direct nursing care to elderly residents in a skilled nursing and rehabilitation setting within LPN scope of practice.
  • Administer medications, treatments, and wound care as ordered, monitoring for effectiveness and adverse reactions.
  • Perform routine patient assessments, including vital signs and basic physical observations, and report changes in condition promptly.
  • Document all nursing care, interventions, and resident responses accurately and timely in the electronic medical record.
  • Support development and implementation of individualized care plans in collaboration with RNs and the interdisciplinary team.
  • Implement and adhere to infection control, safety, fall-prevention, and skin-integrity protocols to protect residents and staff.
  • Educate residents and families about care plans, medications, disease processes, and self-care strategies with empathy and clarity.
  • Coordinate with CNAs and other team members to ensure consistent, high-quality daily care and smooth shift handoffs.
  • Participate in care conferences, quality improvement initiatives, and training programs to enhance patient outcomes and service quality.
  • Promote a respectful, compassionate environment that upholds resident rights, dignity, and person-centered care values.
