Role Overview: The Patient Care Manager - Hospice supports clinical operations in multiple offices, assisting leadership to ensure the delivery of safe, compliant, and quality patient care.
Key Responsibilities- Ensure adherence to hospice policies, procedures, and regulatory standards at local, state, and federal levels
- Promote and oversee quality and safety for patients and staff
- Investigate complaints and resolve issues thoroughly
- Conduct staff in-services on safety, regulatory compliance, and agency policies
- Provide care in diverse home environments, respecting patient diversity
- Unrestricted RN licensure in the practicing state
- Valid driver’s license, vehicle insurance, and dependable transportation
- Preferred: CPR certification, 4+ years of RN experience, 1+ years of clinical practice, independent work capability, strong communication and organizational skills
Compensation & Benefits: Salary ranges from $73,372 to $110,058 annually, supplemented by benefits including health coverage, incentive programs, 401k, and stock options, subject to eligibility.
