Licensed Practical Nurse (LPN) – Medical Records & Infection Prevention

Weakley Nursing & Rehabilitation Center is seeking a detail-oriented and clinically skilled Licensed Practical Nurse (LPN) to serve as our Medical Records & Infection Prevention Coordinator
. This unique leadership role combines clinical oversight, regulatory compliance, infection prevention, and medical records management to help ensure exceptional resident care and survey readiness.

• Maintain accurate, complete, and confidential resident medical records.

• Ensure documentation complies with federal, state, and facility regulations.

• Monitor chart audits and documentation trends for quality and compliance.

• Assist nursing leadership with record requests, audits, surveys, and regulatory reviews.

• Maintain electronic and paper medical record systems.

• Support admissions, discharges, and resident record management processes.

• Lead and coordinate the facility's Infection Prevention and Control Program.

• Monitor, track, and analyze infection surveillance data and trends.

• Conduct infection investigations and implement corrective actions when needed.

• Educate staff on infection prevention practices, PPE usage, and isolation procedures.

• Ensure compliance with CDC guidelines, CMS regulations, and facility policies.

• Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.

• Collaborate with nursing leadership, physicians, and interdisciplinary team members to promote resident safety.

• Current Tennessee LPN license in good standing.

• Long-term care, skilled nursing, rehabilitation, or infection prevention experience preferred.

• Knowledge of infection control regulations and healthcare compliance standards.

• Strong organizational, documentation, and record-management skills.

• Excellent communication and leadership abilities.

• Ability to maintain strict confidentiality and professionalism.

• Experience with electronic medical record systems preferred.
