The Case Manager provides comprehensive, client-centered services to families involved in the child welfare and/or White County Family Treatment Court (WCFTC) system. This position focuses on assessment, service coordination, crisis intervention, and ongoing case monitoring to promote family stability, safety, and self-sufficiency.
Duties include, but are not limited to, the following:
- Complete comprehensive screenings and client assessments to identify strengths, needs, and service priorities.
- Develop, implement, and monitor individualized treatment plans and wrap-around service plans for families.
- Provide intensive case management services, including regular home visits and community-based support.
- Connect participants to community resources such as mental health, medical, substance use treatment providers, housing services, and other support systems.
- Monitor participant compliance with service plans, court requirements, and child welfare case plans.
- Assist families in navigating Department of Children’s Services (DCS) requirements and court expectations.
- Attend and actively participate in DCS and Family Treatment Court hearings, staffings, and team meetings.
- Collaborate with FTC team members, DCS staff, treatment providers, DHS/TANF case managers, and other community partners.
- Provide crisis intervention and short-term supportive counseling as needed.
- Offer educational resources and skill-building support to strengthen family functioning and stability.
- Conduct and monitor drug screenings in accordance with program requirements.
- Maintain accurate and timely client documentation, charts, and case notes.
- Enter required data into the state database and other reporting systems.
- Assist clients with completing disability paperwork and other benefit-related documentation when necessary.
- Maintain ongoing communication with supervisors and funding partners, including FFS Supervisors and DHS TANF Case Managers.
MINIMUM QUALIFICATIONS
- Bachelor’s degree from an accredited college or university; in some cases, relevant work experience may substitute for education.
- 1–2 years of related work experience in case management, social work, or human services.
- Valid driver’s license and reliable transportation.
- Must authorize the release of investigative and criminal records from the FBI and Tennessee Bureau of Investigation to the Tennessee Community Services Agency.
