General Purpose
Handles pressure, objectivity, confidentiality, organization, planning, reporting skills, persistence, proactive, listening, verbal communication, and resident and family relationships.
Essential Duties- Establishes course of action by exploring options and setting goals with residents and their families.
- Assists residents in achieving the highest practicable level of self-care, independence and well‑being.
- Provides medically related social services so that the highest practicable physical, mental and psychosocial wellbeing of each resident is attained or maintained.
- Evaluates social and family information, psychological and emotional needs to assess social services needs and develop care plans for social service issues.
- Provides emotional support and addresses emotional problems, including assisting residents and families with coping difficulties related to physical disabilities, health conditions, fears, grief and loss of ability.
- Assists residents with health‑care decisions.
- Assists with personal shopping.
- Assists in inventory and tracking patient belongings; coordinates responses to reports of missing, lost or stolen belongings.
- Obtains resources from community and social services agencies and health and welfare agencies to meet resident needs.
- Secures appropriate prosthetics and assistive devices.
- Develops and maintains strong working relationships with other departments in the facility and outside community agencies to provide social services.
- Documents resident social service status.
- Assists in making outpatient appointments and schedules on‑site ancillary patient services (optometry, podiatry, dentistry, psychiatric services).
- Assists in discharge planning with appropriate agencies, entities or individuals, including agency services, equipment and referrals.
- Coordinates with the interdisciplinary team.
- Provides in‑service training to staff as requested.
- Counsels residents and family members.
This position has no supervisory responsibilities.
Qualifications- Education:
Bachelor’s Degree in Social Work or Human Services and 2 years of supervised social work experience in a health‑care setting working directly with individuals. MSW preferred. - Language
Skills:
Ability to read technical procedures, read and comprehend policy and procedure manuals, effectively present information and respond to questions from managers and employees. - Mathematical
Skills:
Ability to apply concepts such as fractions, percentages, ratios and proportions to practical situations. - Reasoning:
Ability to solve practical problems and interpret a variety of instructions furnished in written, oral, diagram or schedule form. - Certificates/License:
Knowledge and experience with PCC preferred.
Standing and/or walking very frequently; sitting occasionally; reaching with hands and arms frequently; pushing/pulling very frequently; talking and/or hearing very frequently; tasting and/or smelling very frequently; lifting up to 25 pounds frequently; climbing, balancing, stooping, kneeling or crouching occasionally.
