Clinical Pharmacist – Regional Campus / Ambulatory ServicesBased on a regional hospital campus with strong outpatient integration
Focus on chronic disease management through collaborative drug therapy management (CDTM)
Direct patient care including anticoagulation services (INR testing, therapy monitoring)
Active interdisciplinary collaboration with primary care providers
Clinical Pharmacist – Medical Group / Outpatient ClinicsEmbedded within a multi-specialty physician group and outpatient clinic network
Emphasis on provider collaboration, medication optimization, and patient-facing clinical services
Involvement in drug information, dosing consults, and quality initiatives
Clinical Pharmacist – Pharmacy & Therapeutics (Inpatient, Days)Hospital-based inpatient clinical pharmacist role
Medication order verification and therapeutic review
Participation in P&T, antimicrobial stewardship, medication safety, sepsis, and other committees
Formulary management and performance improvement support
Clinical Pharmacist – Pharmacy & Therapeutics (Inpatient, 7 on / 7 off Evenings)Full-time, 7 on / 7 off evening schedule
Inpatient clinical coverage model
Medication order verification and protocol-based adjustments (renal dosing, IV-to-PO conversion, therapeutic interchanges)
Oversight of IV preparations and injectables
Independent clinical decision-making within approved policies
Compensation & Incentives- Hourly range: $53.37 – $80.06/hr (DOE, role-dependent)
- Annualized range: approximately $111K – $166K
- $20,000 sign-on bonus (paid at hire with a 2-year commitment)

- Provide medication counseling and patient education
- Perform drug information and dosing consultations
- Adjust medication orders per approved protocols
- Participate in interdisciplinary committees (P&T, Antimicrobial Stewardship, Sepsis, Medication Safety, etc.)
- Support chronic disease management through CDTM protocols
- Precept pharmacy students and residents

D from an accredited program - Licensed or eligible for licensure in Indiana
- 1–2 years of residency or equivalent clinical experience preferred
- Board certification preferred within the first year of eligibility
- Experience in acute care, ambulatory, or inpatient clinical settings preferred (depending on role)
