Job Description

An After Hours/ On Call Registered Nurse will provide care during a 7 on 7 off shift.

Schedule:

Mon‑Thurs 5:00 PM–8:00 AM, Fri 5:00 PM–Mon 8:00 AM.

Offering $5,000 Sign On Bonus

7 ON 7 OFF POSITION.

As an After Hours/ On Call Registered Nurse, you will contribute to our success in the following ways:

• Ensure that all activities performed align with the vision of Elara Caring’s board of directors, executive team, and the leadership of the Hospice team.
• Assess hospice care patients and families to identify the physical, psychosocial, and environmental needs of patients as evidenced by documentation, clinical records, Interdisciplinary Team reports, after hours reports, and on‑site evaluations.
• Assess patient needs and obtain data on physical, psychological, social, and spiritual factors that may influence patient/family/caregiver health status.
• Communicate significant findings, problems, and changes in health condition, environment, or unsafe facility conditions to the Clinical Supervisor, physician, facility, and/or other personnel involved with patient care.
• Counsel, instruct, and include the patient, facility, and family in following the Interdisciplinary Plan of Care.
• Teach hospice philosophy and pain and symptom management to staff in alternate care settings (nursing homes, hospital, assisted‑living facilities, etc.).
• Provide appropriate support at time of death and perform bereavement assessment.

What is required:

• Graduate of an accredited Certificate, Diploma, Associate, or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse (RN).
• 1 year of experience in a clinical care setting.
• Experience in a hospice or home health environment is preferred.
• 50% travel required.
• Ability to frequently lift, push, pull, and support up to 50 pounds, including positioning or transferring patients and moving equipment.
• Valid driver’s license and insurance and reliable transportation to perform job tasks.
