Their providers make a dramatic impact in increasing quality of life and safeguarding the dignity of those they serve.

POSITION

DESCRIPTION:

Serve as a primary care provider for geriatric patients located in one or more long-term care facilities / skilled nursing facilities in Terre Haute.

A low volume of patients are seen per day, allowing ample time to focus on all needs and to provide proper attention and support.

Overall duties of the role include, but are not limited to:

  • plan, direct, and evaluate medical care
  • examine and treat chronic illnesses
  • examine and treat urgent illnesses, minor injuries, etc.
  • order and interpret laboratory and diagnostic testing
  • collaborate with caregivers and other healthcare professionals
  • work closely with the interdisciplinary care team
  • make referrals as necessary

The practice uses an EHR on a mobile-based platform and providers are supported by team members who assist with scheduling, orders, referrals, etc.

Providers share on-call responsibilities, which is low volume phone triage only. Providers are on call no more than 3 nights per month and flexibility in that schedule is offered.

REQUIREMENTS:
  • Indiana NP license
  • DEA
  • 1+ year advanced practice experience
  • experience in primary care, geriatrics, acute care, etc.
  • comfort working with medically complex patients
