Hospice Plus is seeking an experienced RN Case Manager to lead patient care in home settings across our hospice program. You will oversee comprehensive assessments, implement skilled nursing interventions, and coordinate with physicians, social workers, chaplains and other team members to ensure compassionate, patient‑centered plans of care.
The role requires active RN licensure and at least 1 year of RN experience, preferably in hospice or palliative care.
