Registered Nurse; RN - Home Health Admission - per visit

Job Overview

BAYADA Home Health Care is seeking a Registered Nurse (RN) Home Health Admission to support home visits throughout Northeast Massachusetts. The role involves completing OASIS Start of Care (SOC) visits for adult and geriatric patients, documenting Medicare‑certified home health care, and working as part of a multi‑disciplinary team to provide skilled nursing and rehabilitative care.

• Use strong head‑to‑toe assessment skills to identify subtle changes in patient condition.
• Utilize experience with acute and post‑acute care (CHF, COPD, wound care, IV therapy, post‑surgical care).
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, teaching, and safety monitoring.
• Perform comprehensive wound assessment, care, and documentation.

• A current nursing license in good standing in the state of Massachusetts.
• Graduation from an accredited and approved nursing program.
• At least one year of recent hands‑on medical experience with adult/geriatric population in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care or rehab units.
• Strong problem‑solving skills in unpredictable home environments.
• Ability to work independently and manage a dynamic visit schedule.
• Strong attention to detail and efficient use of point‑of‑care documentation and technology.
• Comfortable providing care in varied home environments.
• Demonstrates cultural sensitivity and respect for patient autonomy.

Competitive pay ranges with a weekly pay schedule. Weekly base pay: $60–70 per point, depending on qualifications. Flexible scheduling.
