Dierksen Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Lead Responsibilities- Deliver hands‑on hospice, and guide patients and families through every step of their hospice journey.

- Respond to clinical referral information with care and expertise, addressing inquiries and requests with a compassionate touch that showcases our commitment to exceptional service.
- Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services.
- Act as a source of comfort and support for patients, ensuring their comfort and quality of life remain paramount.
- Deliver top‑notch nursing care grounded in proven principles and techniques.
- Educate and advocate for patients and families, encouraging active participation in personalized care plans.
- Share the responsibility of call duties as part of the team.
- Hold a current unencumbered license as a Registered Nurse.
- Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice experience a plus.
- Computer proficiency in typing and various programs, including background in EMR.
- Possess and maintain current CPR certification if required by state.
- Current driver’s license, vehicle insurance, and access to dependable transportation.
$72,000 - $77,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
