Hospice Marketer​/Community Liaison

Overview

Concho Hearts Hospice is looking for an experienced Hospice Marketer in the Texarkana, TX Area. STATE TEXAS SIDE ONLY. Must have 2+ years of Hospice experience.

Position Overview: The Hospice Community Liaison is responsible for establishing and maintaining public relations at facilities, hospitals, and other community businesses.

• Develops various channels of referrals through the selection, acquisition, retention, and growth of business partnerships with referral sources.
• Consistently visits, communicates, and monitors provider, community, customer, payer, and patient perceptions of Concho Hearts Hospice as a high-quality provider of services.
• Conduct educational in-services and community outreach to promote our superior hospice care services.
• Educates patients and their families about hospice expectations.
• Work closely with the clinical operations team to ensure seamless care transition coordination from an acute care setting to a home.

• MUST have Hospice Marketing experience of 2+ years.
