Overview: We are seeking a compassionate and experienced Family Medicine or Internal Medicine Physician to provide comprehensive medical care to adult patients with intellectual and developmental disabilities (IDD) and co-occurring mental health conditions in a residential long-term care setting. This position is responsible for delivering high-quality primary medical care, participating in interdisciplinary treatment planning, conducting psychiatric-related medical evaluations, and responding to medical emergencies within the facility.
The Physician will work collaboratively with nursing, psychiatry, psychology, and interdisciplinary teams to ensure continuity of care.
- Provide comprehensive primary medical care to adult long-term care residents with intellectual and developmental disabilities and dual diagnoses.
- Conduct face-to-face patient evaluations and medical assessments.
- Perform psychiatric-related medical evaluations and support treatment planning in collaboration with behavioral health providers.
- Respond to all on-campus medical emergencies, emergency calls, and restraint situations requiring physician involvement.
- Manage acute and chronic medical conditions while promoting preventative healthcare.
- Complete annual physical examinations, quarterly medical documentation, and all required clinical documentation within established timelines.
- Participate in daily morning rounds and interdisciplinary care planning.
- Attend annual interdisciplinary team meetings and Individual Support Plan Assessments (ISPAs) as required.
- Collaborate with nursing staff, psychiatry, pharmacy, rehabilitation services, and other healthcare professionals to coordinate patient care.
- Maintain accurate, timely, and compliant medical records.
- Complete assigned duties consistent with those performed by permanent physicians at the facility.
- Adhere to agency attendance and leave policies.
- Perform other duties as assigned.
- Provide comprehensive medical care for intellectually disabled adult long-term care residents.
- Residential campus-based practice (no outpatient clinic responsibilities).
- Monday through Friday, 8:00 AM – 5:00 PM.
- Assigned patient panel of approximately 25 long-term care residents located across two residential homes.
- Conduct daily morning rounds by 9:00 AM followed by morning report.
- Average direct patient volume of 1–2 patients per day, allowing time for comprehensive documentation, interdisciplinary collaboration, and care coordination.
- Responsible for:
- Annual physical examinations
- Quarterly medical reviews and documentation
- Interdisciplinary team meetings
- ISPAs (Individual Support Plan Assessments)
- Ongoing clinical oversight and continuity of care
- Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO).
- Board Eligible or Board Certified in Family Medicine or Internal Medicine.
- Active, unrestricted Texas Medical License issued by the Texas Medical Board with no disciplinary sanctions.
- Active U.S. Drug Enforcement Administration (DEA) registration.
- Must be credentialed with hospitals utilized by the facility.
- Current American Heart Association (AHA) Basic Life Support (BLS)/CPR certification.
- Strong clinical judgment with the ability to manage medically complex adult patients.
- Excellent communication and interdisciplinary collaboration skills.
- Experience caring for adults with intellectual and developmental disabilities, behavioral health conditions, or psychiatric populations is preferred.
- Active Texas Medical License (unrestricted)
- Active DEA Registration
- Current American Heart Association CPR/BLS Certification
- Hospital credentialing as required by the facility
- Pre-employment drug screening
- Fingerprinting and background clearance
- Maintain all required certifications and training throughout the duration of employment
