Any individual interested in becoming a substitute Nurse for the Hardin Jefferson
Independent School District is required to have a completed application on file
PRIOR to the district's substitute teacher orientation.
A completed application includes:
* Employment application for substitute Nurse
* Verification of Education (official copy of a public high school transcript or college
transcript)
* DPS CCH Verification Form
* Criminal History Record Information Form (CHRI)
* W-4 form
* Employment Eligibility Form or I-9 (including a copy of your social security card and
driver's license)
* Acceptable User Policy Agreement (AUP)
ALL SUBSTITUTE APPLICANTS MUST BE FINGERPRINTED
* The fingerprinting process will be initiated once your application is received. Each substitute is responsible for the cost of fingerprinting.
Qualifications
A substitute applicant must have one of the following:
* LVN or RN
Substitute Nurse Pay rates:
RN $120/day
LVN $110/day
