Overview
PRN Staff Pharmacist, 2nd shift
At Houston Methodist, the Staff Pharmacist provides day-to-day comprehensive, patient-oriented pharmacy services in all areas of the department. Responsibilities include processing orders, therapy review, and patient and care-team education. The Staff Pharmacist collaborates with healthcare professionals to manage drug therapies for effectiveness and safety, contributes to the department’s mission, and may oversee pharmacy technicians and students, possibly serving as a preceptor to ensure accurate and timely delivery of pharmaceuticals.
QualificationsEducation
- Graduate of an education program approved by the credentialing body for the required credential(s) indicated below in the Certifications, Licenses and Registrations section
Experience
- No experience needed; some experience applicable to the specific work setting (e.g., hospital, specialty pharmacy) highly preferred
Licenses and Certifications
Required
- PHRM - Pharmacist - State Licensure - Texas Department of Licensing and Regulation:
Texas Pharmacist license or eligible for Texas Pharmacist license within 90 days (and) - PPC - Pharmacist Preceptor Cert - Texas Department of Licensing and Regulation: within 15 months (and)
- IV Certification - Intravenous certification (Various) - within 90 days (and)
- BLS - Basic Life Support or Instructor (AHA) - within 90 days
Preferred
- ACLS - Advanced Cardiac Life Support or Instructor (AHA)
- Demonstrates the skills and competencies to safely perform the assigned job, assessed through ongoing skills, competency assessments, and performance evaluations
- Proficiency in speaking, reading, and writing English sufficient for essential functions, especially regarding safety-sensitive activities
- Effectively communicates with patients, physicians, family members and co-workers with a customer-service focus and positive language
- Strong interpersonal, teamwork and leadership skills; coordinates activities of a major pharmacy service area
- Knowledge and skills in unit-dose drug distribution, intravenous admixture, nutrition support services, and target drug monitoring
- Knowledge of rational drug therapy principles and application to all patient groups (neonate to geriatric)
- Computer skills including MS Office and EMR software
- Promotes a positive, team-focused work environment and collaborates with the patient care team; communicates and reports pertinent patient care information comprehensively
- Guides and mentors pharmacy technicians and students; in the absence of senior staff, serves as shift coordinator for technicians and support staff
- Processes all orders/requests accurately and verifies drug appropriateness based on diagnosis, allergies, height and weight
- Monitors workload fluctuations and adjusts technician assignments to ensure timely medication delivery to patient care areas
- Conducts proactive drug therapy reviews, provides drug information, patient counseling, and manages pharmacist-based dosing protocols; collaborates with health care staff for problem resolution
- Collaborates to manage patient drug therapy for effectiveness, side effects, adverse drug reactions, toxicities, interactions and formulary considerations; documents patient care activities and outcomes
- Participates in department quality management activities (e.g., medication use evaluations, adverse drug reactions, medication error reporting, clinical interventions, inventory management)
- Manages inventory to minimize waste and expired drugs; uses resources effectively
- Self-motivated to manage time and priorities daily
- Participates in inter- and intra-departmental activities, including in-services, article publication/presentation, preceptorship, and additional training; seeks opportunities for continual development as a Staff Pharmacist
