Pay Range: $18.50 – $19.50 per hour
Schedule:
Full-Time, Monday – Friday, 8:00 AM – 5:00 PM
Access Health was selected to provide community navigation services as part of the United Way of Greater Houston’s Second Century Vision initiative. Our navigation team helps individuals and families access essential resources and services to meet their personal, financial, educational, and social needs.
The Community Health Navigator serves as a frontline public health worker and acts as a liaison between Access Health clinics and community resources. This role is responsible for connecting patients and community members with healthcare and social services to improve overall well-being.
EssentialJob Duties and Responsibilities
- Represent Access Health at community meetings, events, and collaborative partnerships.
- Assess patients’ social determinants of health and identify needs.
- Provide health coaching and education to patients and community members.
- Assist individuals in accessing resources such as food, housing, health insurance, transportation, and healthcare services.
- Follow up with assigned patients to ensure successful access to care and services.
- Coordinate referrals and connect clients with appropriate external agencies.
- Participate in community outreach events, including health fairs and presentations.
- Accurately document all activities and services in patient records in a timely manner.
- Maintain ongoing communication with patients between visits.
- Serve as a liaison between the clinic and community organizations.
- Participate in ongoing training and professional development opportunities.
- Ensure compliance with organizational policies, procedures, and regulatory requirements.
- Perform additional administrative or related duties as assigned.
- Bilingual (English/Spanish) preferred.
- Experience in community outreach, social services, resiliency, advocacy, or disaster recovery preferred.
- Ability to work both independently and collaboratively within a team.
- Experience with case management or client file management.
- Intermediate proficiency in Microsoft Office (Word, Excel, Outlook) and other systems.
- Professional and effective communication skills when interacting with clients and staff.
- Associate’s or Bachelor’s Degree required (preferred in Public Health, Social Work, or related field).
