Sevita - - Responsibilities:
Deliver support per individual service/program plans; assist with socialization and health maintenance.;
Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community integration, shopping, citizenship, and other activities of daily living.;
Accurately document progress and review records to stay abreast of changes in service plans; maintain confidentiality.;
Report any instance of alleged abuse or neglect and medical/behavioral incidents per policy.;
Monitor health and follow health care directives; assist with medications as assigned and report errors.
