Speech Therapist; ST

Per Visit Rates

• $75 per regular ST Visit
• $112.50 per ST Eval

Fort Worth and surrounding areas.

The Speech‑Language Pathologist is responsible for comprehensive assessments in the patient's home, including the completion of skilled documentation through Homecare Homebase.

• Evaluate patients and perform appropriate therapy interventions
• Develop a comprehensive plan of care and coordinate multi‑disciplinary care
• Perform clinical documentation and visit notes
• Continue communication with physician, care team members, patient and family
• Comfortable working in all aspects of the home health setting
• Implement and maintain SLP protocols
• Manage patient treatment and outcomes directly

• Current Texas Speech‑Language Pathology License
• One year of SLP experience
• Experience with Homecare Homebase is helpful, but not required
• Previous Home Health experience is helpful, but not required
• Current Texas driver's license
• CPR certification
