Community Health Worker (CHW)

Join our dedicated team as a Community Health Worker and play a vital role in improving health outcomes within our community. Under the guidance of the Population Health Director, you will engage in outreach, patient education, and patient engagement activities, working closely with medical providers, primary care teams, and community agencies. This position offers an opportunity to make a meaningful difference by supporting targeted populations and contributing to value-based healthcare initiatives.

Key Responsibilities
  • Conduct outreach activities within the community and assigned clinics to identify and engage target populations.
  • Provide patient education on health management, preventive care, and available resources.
  • Collaborate with healthcare providers and care teams to support patient care plans and improve clinical outcomes.
  • Assist patients in navigating healthcare services and understanding their treatment options.
  • Support data collection and documentation related to patient interactions and health outcomes.
  • Participate in community events and health fairs to promote health awareness and services.
  • Work within the framework of value-based care models to meet specific clinical and health improvement goals.
  • Maintain confidentiality and adhere to all healthcare regulations and organizational policies.
Skills and Qualifications
  • High school diploma or equivalent; additional health-related certifications are a plus.
  • Prior experience in community outreach, patient education, or healthcare settings preferred.
  • Strong interpersonal and communication skills, with the ability to engage diverse populations.
  • Knowledge of local community resources and healthcare systems.
  • Ability to work independently and as part of a multidisciplinary team.
  • Basic computer skills and familiarity with electronic health records (EHR) systems.
  • Bilingual abilities are advantageous but not required.
  • Compassionate, patient-focused attitude with a commitment to health equity.
