Community Health Centers of South Central Texas (CHCSCT) is seeking a dynamic and highly organized Referral Coordinator to join our team at our Gonzales Community Health Center location. As a Referral Coordinator at CHCSCT, you will play a crucial role in ensuring that our patients receive the specialized care they need through an efficient referral process. You will be responsible for managing and coordinating referrals, working closely with healthcare providers, insurance companies, and patients to ensure a seamless and timely access to healthcare services.

Travel Required

Key Responsibilities
  • Verify patient insurance coverage and eligibility for the referred services.
  • Communicate with patients, providers, and insurance companies to ensure timely and accurate processing of referrals.
  • Communicate with patients, educate on the referral process, schedule appointments with referred specialists, and answer any questions and concerns regarding referrals.
  • Obtain necessary authorizations and pre-certifications for specialist visits or procedures.
  • Collaborate with healthcare providers to obtain necessary medical records and documentation referrals.
  • Maintain accurate and up-to-date records of referrals and follow-up actions.
  • Ensure that referrals are processed in compliance with healthcare regulations and company policies.
  • Monitor and report on referral performance metrics to identify opportunities for improvement.
Qualifications
  • High school diploma or equivalent.
  • Previous clinical referral experience.
  • Strong organizational skills with the ability to prioritize tasks effectively.
  • Excellent communication skills, both written and verbal.
  • Knowledge of medical terminology and insurance processes is a plus.
  • Proficiency in using computer applications and electronic medical records.
  • Ability to work well in a fast-paced environment while maintaining attention to detail.
  • Ability to speak Spanish/English, preferred.
