• Conduct initial assessments and evaluations of patients.
• Develop and implement personalized treatment plans.
• Document patient progress and maintain accurate records using Homecare Homebase.
• Participate in weekly case conferences to discuss patient progress.
• Provide routine visits, 30-day re-assessments, and coordinate discharge plans.

• Home Health and Oasis experience required.
• Homecare Homebase documentation experience necessary; training can be provided for the first two weeks at a reduced rate.
• Current Physical Therapist license required.
